The Community-led Advance Care Planning programs for people living with dementia and family caregivers project, led by the BC Centre for Palliative Care, has created this document to help you engage older adults and individuals living with dementia in advance care planning. This tool includes questions which can be used to help start a conversation with individuals about Advanced Care Planning to understand where they are in the process and how you can provide support.
